and complete the Parents’ Financial Statement (PFS). The Ellis School code is 2974. There is a $51.00 fee, which must be paid or waived (this process occurs automatically if you qualify for a fee waiver). This fee covers multiple reports to participating schools and will be processed via debit card, credit card, or PayPal.REQUIRED DOCUMENTS CHECKLISTIn the application, you will provide information from fiscal year 2018 and projections for 2019. Upon completion of the application, you will be asked to upload your 2017 tax documents; your financial aid application will not be complete until you have uploaded your 2017 IRS forms. We realize that most tax forms for 2018 are not available until late January, so we recommend that you refer to your 2017 tax return to estimate your 2018 information. Please do not delay your application until you have your 2018 tax documents if that will cause you to miss the deadline (December 15, 2018 for current Ellis families; January 15, 2019 for prospective Ellis families). If you miss the deadline, your application will be considered on a wait-list basis. You are required to submit your 2018 tax forms to SSS as soon as possible and no later than April 20, 2019. Without these forms, your financial aid will be suspended until your application is updated.Due by application deadline:

- 2017 W-2 Wage and Tax statements
- 2017 Federal 1040 or 1040A
- Copies of all supporting tax schedules
- Business – Schedule C or C-EZ (1040)
- Rental Property – Schedule E (1040)
- Business and Farm Owners: Business/Farm Statement for each business or farm owned with a copy of the applicable 2017 tax return (form available on the SSS website)
You have the option of submitting the required documents to SSS by uploading them online or by mail. If you submit them by mail, send them to the following address along with the Required Documents Cover Sheet, which is available online: School and Student Services; PO Box 449; Randolph, MA 02368-0449.For the purposes of establishing financial need, Ellis expects that both parents (and all legal guardians) will contribute to their daughter’s education. In the case of divorced or separated parents, both parents must complete the Parents’ Financial Statement (PFS) and submit all the required documentation to School and Student Services (SSS). Financial information is required from both custodial and non-custodial parents. In certain circumstances, (for example, a widowed parent) a sole parent or guardian can submit
a statement by a third party in a professional capacity who is aware of the situation. For new aid applicants, all financial documents from each parent or a statement of non-involvement must be submitted before the financial aid application will be processed.FOR MORE INFORMATION
